Information for Health and Social Care Professionals

St Luke’s Hospice is a registered charity offering a range of specialist palliative care services to people with any life threatening, life limiting disease, predominantly but not exclusively cancer.

All Hospice staff work closely with a wide range of health and social care professionals to achieve the highest level of care for patients and their families.

Referrals
Referrals can be accepted to Hospice services from any health or social care professionals and also from patients and families where appropriate.

Referrals are regularly discussed and are considered on the basis of need.

Referrals can be made by completing a referral form which can also be obtained from the Hospice, completed and then faxed as follows:-
