Information for Health and Social Care Professionals St Luke’s Hospice is a registered charity offering a range of specialist palliative care services to people with any life threatening, life limiting disease, predominantly but not exclusively cancer. All Hospice staff work closely with a wide range of health and social care professionals to achieve the highest level of care for patients and their families. Referrals Referrals can be accepted to Hospice services from any health or social care professionals and also from patients and families where appropriate. Referrals are regularly discussed and are considered on the basis of need. Although working independently as a charity, referrals made by completing the South West Essex NHS Single Referral Form which can also be obtained from the Hospice, completed and then faxed as follows:-
Please do not e-mail patient information Education and Professional Development Services We have over 20 years of experience in caring for patient and families, our Education Team facilitate evidence based Palliative care training to the multi professional team working in palliative care or generic settings. We provide learning opportunities in palliative care that will enhance and improve end of life care for patients across the community. For more information click here |
