Professional, Community and Business Partnerships Clinical Partnerships St. Luke's Hospice has developed effective working partnerships with other palliative care providers in order to provide increased and seamless services to people in our local community. St. Luke's works in tandem with these services in a variety of ways, which may include providing a venue and/or providing staff for a service. These partnerships include:
Macmillan Dove This service is a partnership between Macmillan Cancer Relief and Thurrock MIND. More >> South Essex Lymphoedema Service (SELS) This service is a South Essex wide service for patients with cancer related lymphoedema. The clinics are held at St Luke’s on Thursdays and Saturdays. Occupational Therapy/Physiotherapy Services These services aim to maximise a patients physical function and mobility to enable patients to live their lives as fully as possible through detailed assessment and advice on lifestyle management, thus improving quality of life. specialised equipment is available to meet varying needs and individual patient assessment/care plans are tailored to each patient's needs/requirements. Occupational Therapy These specialist practitioners are part of the Community Palliative Care Team. Appointments are available through referral to the service and also through referral to the Outpatient Clinic. Physiotherapy Our Physiotherapist is available twice a week to support patients. Individual appointments are available including through attending the Outpatient Clinic. This can be particularly beneficial for people who need to maintain mobility such as people suffering from Multiple Sclerosis or Motor Neurone Disease, for example. Hospice at Home This service is a partnership between St. Luke's Hospice and SPDNS Nurse Care. More >> Community Macmillan Palliative Care Team St. Luke's Hospice has a full time Macmillan Nurse working within the Community Macmillan Palliative Care Team. They are a team of qualified nurses with specialist knowledge of palliative care and cancer whose principle role is to provide advice, support and information. As a team of specialist nurses they visit patients in their own homes, to help them remain as active and as well as possible so they can remain at home if that is what they and their families/carers wish.
