I confirm that I am a UK tax payer and I would like St. Luke’s Hospice to reclaim the tax on all donations I have made in the last 4 years, and on any future donations, unless I notify you otherwise. I have paid enough UK Income Tax or Capital Gains Tax to cover all of my charitable donations and I know that Council Tax and VAT do not qualify.

Please let us know if your circumstances change. If you do not pay the same amount, or more, of UK Income Tax and/or Capital Gains Tax then it is your responsibility to pay any difference.
