St. Luke’s Hospice is a registered charity offering a range of specialist palliative care services to people with any life threatening, life limiting disease, such as cancer, Heart Failure, Advanced Liver Disease, End stage Renal Disease, Multiple Sclerosis, Chronic Obstructive Pulmonary Disease or Motor Neurone Disease.

Care and support is offered through a wide range of services that are personalised to meet the needs of individuals. This care and support is extended to carers, families and friends.

Services are for people aged 16 years and upwards except were stated otherwise, (e.g. Luke’s Counselling and Support for Children and Young People)

All Hospice staff work closely with a wide range of health and social care professionals to achieve the highest level of care for patients and their families.

Operating 24 hours a day, 7 days a week our OneResponse team is a co-ordinating service offering support and advice for people with palliative care needs, or people needing care at end of life. It is a central point of communication for patients, their carers’ and family as well as for professionals. The team will work with other Hospice colleagues to ensure that you are accessing the right service in the Hospice to meet your needs.

Referrals

Anyone living in the Basildon and Thurrock districts can access Hospice services. Referrals can be accepted from any health or social care professional involved in the persons care and also from patients and families where appropriate. Referrals are discussed every day (weekdays) and are considered on the basis of need. Urgent referrals at weekends can be made by contacting the One Response Team on the telephone number above.

The South Essex Lymphoedema Service provides services to people living in South Essex including, Thurrock, Basildon, Billericay, Wickford, Southend, Canvey, Castlepoint and Rochford.

Although working independently as a charity, referrals are made by completing the South West Essex NHS Single Referral Form which can also be obtained from the Hospice and submitted by email on [email protected]  

Education and professional development services

Our Education Team facilitate evidence based Palliative care training to the multi professional team working in care settings. We provide learning opportunities in palliative care that will enhance and improve end of life care for patients across the community.
