Anyone living in Basildon and Thurrock can access St. Luke's Hospice services.

Referrals are welcomed from all health and social care professionals involved in a person's care, as well as directly from a patient and families.  Referrals are discussed every day (weekdays) and are considered on the basis of need. Urgent referrals at weekends can be made by contacting the One Response Team on the telephone number 01268 526259.

The South Essex Lymphoedema Service provides services to people living in South Essex including, Thurrock, Basildon, Billericay, Wickford, Southend, Canvey, Castlepoint and Rochford.

Although working independently as a charity, professional referrals can be made by completing the South West Essex NHS Single Referral Form which can also be obtained from the Hospice and submitted by email on [email protected]  Should you require an accessible version of the referral form, please contact the Hospice team directly on 01268 524 973.
