Your care will be provided and co-ordinated by a dedicated, specialist Hospice team.

Listening to you and your loved ones, and working together with other health and social care providers and those already involved in your care, we can ensure your quality of life is the best it can be, and your wishes and wants are respected.

Two thirds of Hospice community patients never visit St. Luke’s Hospice and are able to experience specialist care and support where they live, including care homes.

Our care teams include numerous specialists such as Clinical Nurse Specialists (CNS) and Palliative Care Consultants, who provide round-the-clock support, 365 days a year, offering a multitude of services across the communities we serve, including:

  • 24 hour advice and support telephone line, OneResponse
  • planned respite sessions, assessment of patient needs and over the phone support
  • night support, working in liaison with Marie Curie nurses, to provide reassurance and advice
  • crisis support to help prevent an unwanted admission to hospital
  • rapid assessment and discharge services to assist and manage hospital discharges and referrals into community care, for those patients whose condition is rapidly deteriorating or complex.
  • support for care home staff to assist with frail patients, as well as managing discharges from hospitals into care homes and for those who are transitioning between care settings,
  • supporting patients’ choice and care at their preferred place of care at the end of life.

Referrals
